A Comparative Analysis of the Beneficially Effect of the Two-Trocar Laparoscopic Cholecystectomy

Abstract
|

|
Purpose: A new technique for laparoscopic cholecystectomy using only two trocars with the assistance of retraction sutures has been utilized with the goal to minimize further surgical invasiveness. The clinical effectiveness of this technique was evaluated in comparison with the use of the conventional four-trocar technique.
Methods: Forty-two patients who underwent a two-trocar laparoscopic cholecystectomy and 42 patients who underwent a conventional laparoscopic cholecystectomy were compared in a prospective study between January 2006 and September 2006. Patients with severe inflammatory cholecystitis were excluded from the study population. Variables such as operation time, complications, conversion to an open procedure, hospital days and satisfaction grades were compared.
Results: There were no complications for patients undergoing either procedure. There was one case where a patient that underwent the two-trocar procedure ultimately underwent a conversion to the use of the conventional four-trocar technique, but there were no cases where a patient underwent a conversion to the use of an open cholecystectomy. For operation time and hospital stay there were no significant differences for patients
that underwent either procedure. Patients who underwent the two-trocar procedure were nearly satisfactory in terms of
cosmetic effect as well as pain alleviation.
Conclusion: A two-trocar cholecystectomy is technically feasible and provides more satisfaction for cosmetic effect and pain alleviation as compared to the use of the conventional four-trocar procedure. Therefore, the use of the two-trocar cholecystectomy is recommended for well-selected patients.
